Come to this practical session to learn how to have a voice in your health care through advance care planning, how to help family and friends start the advance care planning process, and about some easy-to-use, step-by-step tools – from Dr. Sudore's free PREPARE program – that can assist you with this process. Time will be saved for questions and discussion.

Rebecca Sudore, MD, is a geriatrician, palliative medicine physician, and clinician investigator. Dr. Sudore also directs the Innovation and Implementation Center in Aging & Palliative Care (I-CAP) and the Vulnerable Populations Aging Research Core of the NIA-funded Pepper Center.
